Which is more effective - cleaning hands with alcohol-based hand rub or washing hands with soap and water?

They provide similar disinfection effectiveness but the alcohol-based hand rub is more effective at killing germs. If your hands are visibly soiled or contaminated with respiratory secretions, washing hands is the only option. In this case, you have to wash your hands with soap and clean running water for 20 seconds.
